Safely providing the COVID-19 vaccine requires your utmost cooperation.
To minimize Coronavirus exposure, we have implemented the following safety protocols:
1) It is important that you download and complete this Patient Registration Form.
You must provide the completed form when you arrive for your vaccination.
2) Appointment times cannot be changed or rescheduled as we receive limited inventory from the State that must be used immediately. Failure to arrive on time will result in the automatic cancellation of your appointment. Your second dose will automatically be scheduled at the same time 3-4 weeks after the first dose if a single dose vaccine is not administered.
3) In order to practice infection control, only patients with an appointment will be admitted into the office. Patients must wear a mask at all times while inside the office. Patients will be restricted to the modified waiting room only with no restroom access. After the vaccination, you will be observed for 15 minutes. In order to prevent contagion spread, we ask that you refrain from talking while in the office.
